GROOMING RELEASE FORM FOR ELDERLY INFIRM, SERIOUSLY MATTED PETS AND SPA POLICY

La Bone Adventure Pet Spa requires that all pets being groomed is current on all their vaccinations, including (for dogs): DHPPC, Rabies, Bordetella, and a Fecal Exam. Cats are required to be up to date on Rabies, Feline Distemper, Feline Leukemia, and Fecal Exam.
La Bone Adventure Pet Spa will not responsible for any accidental death of the pet(s) due to the nature of any pre-existing health condition. We also will not be held responsible for minor nicks resulting from the grooming of matted, neglected coats or for irritation caused by removing coat from pets possessing mild to severe skin allergy, nor will we be held responsible for stressful effects that grooming may have upon an elderly pet.
DEMATTING AND EXCESSIVE UNDERCOAT:
Removal will be charged at the rate of $20.00/per hour in addition to regular grooming charges. We, as ethical groomers, reserve the right to refuse to demat a pet UNLESS we deem that the pet can tolerate the amount of dematting and that it could be done humanely.
FLEAS AND TICKS:
Any pet (s) found to be contaminated with fleas or ticks will also be treated at the owner’s expense.
**Pet owners may be required to sign a release for senior pet(s) and /or for very matted pet(s), if the groomer deems it is necessary.
SOILING ACCIDENTS AND AGGRESSIVE BEHAVIOR:
Please take or let your pet(s) out for its “morning duties” before he/she comes for grooming. This will help alleviate any “accidents” upon arrival, during or after grooming. If it is rainy that morning, please take your pet out early enough so that he/she is fairly dry upon arrival here. Additional charges may be incurred when a pet is very difficult, exhibits risky behavior (biting, snapping, and intolerance to grooming in general) or has “soiling” behavior. These behaviors increase the amount of time that it takes to groom, reclean or regroom, your pet(s) Please take or let your pet(s) out for its “morning duties” before he/she comes for grooming. APPOINTMENTS AND WALK-INS:
Appointments and cancellations must be made at least 24 to 48 hrs in advance. You can call us at (954)-680-0668. Walk-ins are accepted from Monday-Thursday only. Restrictions may apply. We do not accept walk-ins on Saturday. If you would like a Saturday appointment you must call and make an appointment no later than Thursday. Pets requiring full grooming services must wait 3 to 4 hrs for pick up or until called depending on the condition of your pet. We ask that you pick up your pet before our closing time of 6:00 pm, if you are late picking up your pet you will assess an additionalÂ fee of $20.00 per half hour.
PICK UP AND/OR ABANDONMENT OF PET:
If a grooming client is not picked up by closing time (6:00 PM), the pet will be boarded at the rate of $65.00 /per night per dog until released to owner. If the pet is not called for within fifteen (15) days of stated time of return, La Bone Adventure Pet Spa has the right to find in a suitable home for the pet) within ten (10) days after this period. The owner will receive one (1) notification by registered mail during this period…no further notice will be given.
DENIAL OF SERVICES:
We reserve the right to charge additional fees for services we consider over and above the norm covered by our standard rates. We also reserve the right to refuse service to customers whose pet may pose a threat to us or to the other pets left in our care, whether it be an aggression problem, health problem, or parasite problems (other than fleas).
Pet owners agree to be responsible for any property damages by pet(s). This includes damages to ANY property from releaving themselves in the store.
